If you suffer from an illness or other medical condition that prevents you from working, you may be considered disabled under Social Security Administration (SSA) rules and eligible for Social Security Disability Insurance (SSDI). You can begin the process of obtaining disability benefits by completing an application for benefits.
Information You Will Need to Apply for SSDI
You will need to gather information before you start completing your application. Here is some of the information that you will need:
- Your age and Social Security number
- Contact information from your doctor and other healthcare providers and the dates they treated you
- Medication names and dosages
- Your medical records for treatments you received for your disability
- Laboratory and diagnostic test results
- Summary of where you worked and your job duties
- Most recent W-2 or your last year’s tax return if you were self-employed
- Social Security numbers and proof of age for family members who qualify for benefits
- Proof of your current marriage if your spouse qualifies for benefits
- Your bank account information
How to Apply for Social Security Disability Insurance Benefits
There are three ways that you can apply for SSDI:
- Complete an online application
- Call the SSA at 1-800-772-1213
- Call or visit a local SSA office
If you want to complete your application in person, you should call the SSA to schedule an appointment.
If you file online, you will find that the Adult Disability Report and Work History Report have been included in the application. You will need to provide detailed information on your medical condition, your doctors, hospitalizations, and treatments you received in the Adult Disability Report. At the end of the application, you will also be asked if you want to give the SSA the authorization to directly obtain your medical records.
Once you complete your application, you should periodically check on its status. This is important to be certain that you provide all the information the SSA needs to make a decision on your application and that the SSA is working on your case.
